Important Things to Remember for 2012-2013

Jacksonville ISD Cafeteria plan year is September 1, 2012 through August 31, 2013.

The Third Party Administrator (TPA) for the 125/129 Plans for Jacksonville ISD for plan year September 1, 2012 to August 31, 2013 is “NBS” (National Benefit Services LLC).

To all CAFETERIA/FLEX Participants: At the end of the plan year (8/31/2012), PLEASE do not throw away your debit flex card. Your new contribution amount for the next plan year will be updated on your existing Flex Card. You must re-enroll every year, as the flex account does not roll-over to the next plan year. Forms are available on-line if you prefer to mail in your claims or if the vendor does not accept the NBS Flex Card. Save your receipts for substantiation if required. IMPORTANT: These accounts are “USE IT OR LOSE IT.” Plan your contributions carefully! For additional information refer to the Benefits & Forms tab / Reimbursement plan.

DENTAL: Met Life is the dental provider for Jacksonville ISD employees, effective 9/1/12. There are NO plan changes effective 9/1/2012. For plan and rate information, click on the Benefits & Forms tab / Dental.

VISION: Block Vision is the vision provider for Jacksonville ISD employees, effective 9/1/12. For plan and rate information, click on the Benefits & Forms tab / Vision.

DISABILITY: Unum Provident Educator Select is the disability provider for Jacksonville ISD employees, effective 9/1/12. All employees can enroll in the plan without evidence of insurability this year (12 month pre-existing condition exclusions will apply). CANCER: American Public Life (APL) is the cancer plan provider for Jacksonville ISD employees, effective 9/1/12. The cancer plan is portable for employees leaving the district. For plan and rate information, click on the Benefits & Forms tab / Cancer.

LIFE: Dearborn National is the group term life and AD&D provider for Jacksonville ISD employees. Rates have increased for the Dearborn VTL plan this year. For plan and rate information, click on the Benefits & Forms tab / Life.
