Important Things to Remember for 2009-2010
IMPORTANT: ALL HEALTHCARE REIMBURSEMENT PARTICIPANTS WILL RECEIVE A NEW FLEX CARD FOR THE PLAN YEAR BEGINNING 9/1/2009. “BENNY” FLEX CARDS ARE NO LONGER ACTIVE AS OF THE LAST DAY OF THE PRIOR PLAN YEAR, 08/31/2009. CLICK ON BENEFITS AND FORMS/REIMBURSEMENT ACCOUNTS FOR MORE INFORMATION.
- Jacksonville ISD Cafeteria plan year is September 1, 2009 through August 31, 2010.
- For NEW participants of the 125 Medical Reimbursement Plan you will receive 1 MGM Flex Card. If you are a continuing flex plan participant, you will receive a new MGM Flex Card. At the end of the plan year (8/31/2010) PLEASE do not throw away your card. Your elected contribution amount for the next plan year will be updated on your existing Flex Card. For replacement of a lost/misplaced card the fee will be $10. Forms are available on-line if you prefer to mail in your claims or if the vendor does not accept the MGM Flex Card. - MGM is the Third Party Administrator (TPA) for the 125/129 Plans for Jacksonville ISD for plan year September 1, 2009 to August 31, 2010.
- Dental: Met Life is the dental provider for Jacksonville ISD employees, effective 9/1/09. There will be a rate increase this year. For plan and rate information click on the benefit highlights tab/dental.
- Vision: Block Vision is the vision provider for Jacksonville ISD employees, effective 9/1/09. For plan and rate information click on the benefit highlights tab/vision.
- Disability: Unum Provident Educator Select is the disability provider for Jacksonville ISD employees, effective 9/1/09. All employees can enroll in the plan without evidence of insurability this year (12 month pre-existing condition exclusions will apply). For plan and rate information click on the benefit highlights tab/disability.
- Cancer: American Public Life (APL) is the cancer plan provider for Jacksonville ISD employees, effective 9/1/09. For plan and rate information click on the benefit highlights tab/cancer.
- Life: Fort Dearborn is the group term life and AD&D provider for Jacksonville ISD employees. For plan and rate information click on the benefit highlights tab/life.
- Supplemental plan offerings to Jacksonville ISD employees for 2009-2010 plan year include: Met Life Dental Plan, Block Vision, Unum Educator Select Disability Plan, American Public Life Cancer Plan, Fort Dearborn Voluntary Term Life and AD&D, and Flexible Spending Accounts.
