Important Things to Remember for 2011-2012
Jacksonville ISD Cafeteria plan year is September 1, 2011 through August 31, 2012.
NEW! The Third Party Administrator (TPA) for the 125/129 Plans for Jacksonville ISD for
plan year September 1, 2011 to August 31, 2012 is “NBS” (National Benefit Services LLC).
This year ALL participants of the 125 Medical Reimbursement Plan will receive 1 NBS Flex Card. Please make sure your address is correct in the enrollment system or you will not receive your card. If you need additional cards for your spouse or college age child you may order those from NBS at no cost. NOTE: To all participants, at the end of the plan year (8/31/2012) PLEASE do not throw away your card. Your elected contribution amount for the next plan year will be updated on your existing Flex Card. You must re-enroll every year, as the flex account does not roll-over to the next plan year. For replacement of a lost/misplaced card the fee will be $10. Forms are available on-line if you prefer to mail in your claims or if the vendor does not accept the NBS Flex Card. Save your receipts for substantiation if required. IMPORTANT: These accounts are “USE IT OR LOSE IT.” Plan your contributions carefully! The MGM card that you currently have will expire on 8/31/2011. New “NBS” Flex cards will be sent to your home address the second week of September 2011, if you have updated your address in the system.
Dental: Met Life is the dental provider for Jacksonville ISD employees, effective 9/1/11. There are NO plan changes effective 9/1/2011, but the rates have increased this year. For plan and rate information, click on the Benefits & Forms tab/dental.
Vision: Block Vision is the vision provider for Jacksonville ISD employees, effective 9/1/11. For plan and rate information, click on the Benefits & Forms tab/vision.
Disability: Unum Provident Educator Select is the disability provider for Jacksonville ISD employees, effective 9/1/11. All employees can enroll in the plan without evidence of insurability this year (12 month pre-existing condition exclusions will apply). For plan and rate information, click on the Benefits & Forms tab/disability.
Cancer: American Public Life (APL) is the cancer plan provider for Jacksonville ISD
employees, effective 9/1/11. The cancer plan is portable for employees leaving the district. For plan and rate information, click on the Benefits & Forms tab/cancer.
Life: Dearborn National is the group term life and AD&D provider for Jacksonville ISD employees. Rates have increased for the Dearborn VTL plan this year. For plan and rate information, click on the Benefits & Forms tab/life.
Supplemental plan offerings to Jacksonville ISD employees for 2011-2012 plan year include: Met Life Dental Plan, Block Vision, Unum Educator Select Disability Plan, American Public Life Cancer Plan, Dearborn National VTL and AD&D, and Flexible Spending Accounts.
