IMPORTANT: Effective beginning on January 1, 2011, over-the-counter
medicines or drugs (e.g. Advil, Ibuprofen, cough syrup) are not eligible for reimbursement under an
FSA, without a doctor’s prescription. Insulin is the only medicine that doesn’t require a prescription.
Also supplies that you need for medical care (e.g. contact lens solutions, bandages for wounds, thermometers)
will continue to be eligible for reimbursement.
As such, upcoming enrollment in your employer’s flexible spending account
should take into consideration as of 1/1/2011, over the counter items will no longer
be reimbursable under the health FSA.

Important Things to Remember for 2010-2011
- Jacksonville ISD Cafeteria plan year is September 1, 2010 through August 31, 2011.
- For NEW participants of the 125 Medical Reimbursement Plan you will receive 1 MGM Flex Card.
NOTE: To all participants, at the end of the plan year (8/31/2011) PLEASE do not throw away
your card. Your elected contribution amount for the next plan year will be updated on your
existing Flex Card. You must re-enroll every year, as the flex account does not roll-over to
the next plan year. For replacement of a lost/misplaced card the fee will be $10. Forms are
available on-line if you prefer to mail in your claims or if the vendor does not accept the MGM
Flex Card. Save your receipts for substantiation if required. For additional information refer

Important: These accounts are "USE IT OR LOSE IT."

- Dental: Met Life is the dental provider for Jacksonville ISD employees, effective 9/1/10. There
will be a rate increase this year. For plan and rate information click on the Benefits & Forms tab/dental.
- Vision: Block Vision is the vision provider for Jacksonville ISD employees, effective 9/1/10.

- Disability: Unum Provident Educator Select is the disability provider for Jacksonville ISD employees,
effective 9/1/10. All employees can enroll in the plan without evidence of insurability this year (12 month pre-existing

- Cancer: American Public Life (APL) is the cancer plan provider for Jacksonville ISD employees, effective 9/1/10. NEW!
This year the cancer plan is offering “portability” for employees leaving the district.

- Life: Fort Dearborn is the group term life and AD&D provider for Jacksonville ISD employees. For plan and rate

- Supplemental plan offerings to Jacksonville ISD employees for 2010-2011 plan year include: Met Life Dental Plan,
Block Vision, Unum Educator Select Disability Plan, American Public Life Cancer Plan, Fort Dearborn Voluntary Term Life
and AD&D, and Flexible Spending Accounts.
